Assets minus liabilities

Assets are things of value: cash, brokerage and retirement accounts, your home, vehicles, and anything else you could sell. Liabilities are what you owe: your mortgage, auto and student loans, and credit card balances. A home counts at its market value with the mortgage subtracted as a separate debt, so a $350,000 house with a $250,000 loan adds $100,000 to your net worth, not the full price.

Should I include my home and car?

Yes, at their current market value, with the loan against them counted as a liability. Some people also track a more liquid figure that leaves out the home and car, since those are harder to spend. Both views are useful, and Telemetry can show either.
